Referrals & Fees
Referral Process and Contacts
Individuals can be referred to PRS in a variety of ways including the following referral sources:
- Mental health provider (i.e., therapist, case manager, psychiatrist, residential counselor, etc.)
- Rehabilitation Counselor from the Department of Rehabilitative Services (DRS)
- Family member or friend
- Self referral
- Managed Care Organization
- A completed PRS referral form and documentation of diagnosis and medications is required.

Services & Fees
Recovery Academy Program
Units are calculated as follows:
Up to 3.99 hours = 1 unit, 4.00 – 6.99 hours = 2 units
$70.00 per unit
Recovery Academy Assessment
$325.00 per assessment (Payable at intake)
Employment Intake Assessment
$185.00 per assessment (Payable at intake)
Work Adjustment Training (WAT)
Units are calculated as follows:
Up to 2.99 hours = 1 unit, 3.00 – 6.99 hours = 2 units
$95.00 per unit (maximum of two units per day)
Employment Services (excluding WAT)
$95.00 per hour
Community Support Services
$120.00 per hour
Outpatient Therapy Services
$135.00 per hour
Family Peer Support Partner Services
$70.00 per hour
Transition to Independence Process (TIP)
$325.00 per assessment (Payable at time of assessment)
$1,200 per month (case rate)
