This is not a job placement service, where we walk away after you are employed. Instead, we partner with you to prepare for, choose, get, and keep a job. We start with you – your goals and strengths. Together, we create your plan. Then, we support you through your next steps – and stay available – for as long as we are helpful to you.

HopeLink providers are trained in the “Choose-Get-Keep” model of individual-supported employment, ensuring that you receive services tailored to your needs, abilities, and goals. HopeLink can help you with:

    • Identifying your work skills, aptitudes, values, and desires
    • Setting realistic job expectations
    • Job search support
    • Preparing to interview for a position
    • On- and off-the-job assessments
    • Coping with symptoms on the job
    • Specialized services for your professional and technical skills

If this sounds like the program for you or someone you know, submit a referral form to get started. If you have any questions or need assistance with the referral form, please contact us.

Referrals

Individuals may be referred to HopeLink in several ways, including through:

    • Mental health provider (i.e., therapist, case manager, psychiatrist, residential counselor, etc.)
    • Rehabilitation Counselor from the Department for Aging and Rehabilitative Services (DARS)
    • Family member or friend
    • Self-referral

Referrals should be directed to the Regional Director of Supported Employment at the geographically appropriate center, as listed below.

Referrals from mental health providers or DARS Counselors should include a HopeLink written referral form, with documentation of diagnosis and medications.
