This is a life skills training service, where you meet in your home or other community settings with your HopeLink provider. You will learn and practice life skills to support the management of your symptoms and improve your daily activities. You will also be encouraged to address any physical healthcare issues you may be facing.

Services are tailored to support your individual needs and goals. Services support individual needs and the development of skills to cope with multiple disorders.

MHSS are person-centered, recovery-oriented, and strengths-based. You connect with one of our HopeLink providers to:

    • Learn or improve daily living skills and increase self-sufficiency to live independently in the community.
    • Learn or improve home maintenance skills to reduce risk of eviction from housing.
    • Make informed decisions and become invested in recovery.
    • Improve social skills and use of leisure time for better overall wellness and resilience.
    • Improve collaboration with healthcare providers, family members, and/or others important to you to ensure that comprehensive service needs are met.

If this sounds like the service for you, submit a referral from your mental health practitioner so we can confirm eligibility and next steps. If you have any questions or need assistance with the referral form, please contact us.
