Transition to Independence Process (TIP)

Building on Your Strengths to Create Your Best Life

The Transition to Independence Process (TIP) Model® is an evidence-based approach to supporting youth and young adults (14 – 29 years old) with emotional or behavioral difficulties in successfully transitioning to adulthood. TIP operates with minimal exclusionary criteria, welcoming those with mental health, substance abuse, and/or mild developmental disabilities; and/or involvement with agencies such as children’s protective services, juvenile justice, or public assistance, and/or youth who are runaways, homeless, or otherwise at risk of transition to adulthood difficulties.

This youth-driven program uses a team of providers to “wrap around” you as you build independent living skills. We work with you to create a custom plan that starts with your strengths, needs, and goals. Our team is here to walk with you, so you can transition well into adulthood and live your best life.

Through TIP, you can find support to:

    • Increase your employment and post-secondary career education
    • Improve your community-life functioning
    • Reduce your use of intensive mental health services and avoid incarceration

Your team of providers will support your transition by:

    • Engaging you in your own future planning process
    • Providing you with developmentally appropriate, non-stigmatizing, culturally competent, and appealing services and supports
    • Involving you and others in your life in the preparation process
    • Facilitating your movement toward greater self-sufficiency and successful achievement of your goals
